There are two main ways to arrive to Zapotitlan de Mendez, where the Karmidas cavern is located: from Zacapoaxtla or from Zacatlan, through the Interserrana road, whoch is completely paved.
From Mexico City you can arrive to Zapotitlan via Zacatlan, through Tulancingo or Apizaco, in 5 or 6 hrs; from Puebla you can arrive via Zacapoaxtla (4.5 hrs) or via Zacatlan (5 hrs).
The intense rains that occured in October damaged seriously the roads of the sierra, the region was incomunicated during two weeks; however, the access is now open.
